Birthday Party Waiver Forms
Must be filled out my all party guests and turned in at the start of the party. Only children with this form will be allowed in the gym. No exceptions!
The Circuit Gym Waiver Release Form
This needs to be filled out for The Circuit gym when your child is doing one of our non-school day camps where we will do a walking field trip to their gym. If you already have a waiver on file with them you do not need to fill this out again.
